**YOUTH FALL STREET(Ball) HOCKEY REGISTRATION FORM**
September 24-December 20th, 2007
Registration Begins: August 1, 2007
Age Groupings: (Circle one) 7-9, 10-12, 13-15.
Minimum 4 teams needed for each age group. 12 players on a team. Game is played 5 v 5 plus a goalie.
Cost: $100.00 per player. Cost of a jersey is $10.00
Length of Season: 10 games plus playoffs for all age groups, with practices during the season based on availability. One pre season practice time is guaranteed.
Equipment Required: Sneakers (tennis shoes), Hockey Stick, Shin Guards, Gloves, Helmet with Full Cage. Games are played with a ball indoors.
Registration can be paid for by cash or check only.

Games will be played on Sundays from 5:00-9:00 P.M. Games will last 1 hour.
CONSENT TO PARTICIPATE
I understand that participation in this sports league involves risk of injury. I also agree to follow all the rules and procedures and to follow the instructions of the league. I agree to waive any legal rights I may have to seek payment of any kind from the owners of the Dream Sports Center, referees or staff for bodily injury.
